Postion Overview
As a Care Coordinator at Starlight Support Services, you will be an essential part of our team, ensuring the delivery of high-quality care and support to our clients. You will coordinate care plans, liaise with healthcare professionals, and ensure our clients receive the services they need. Your role is vital in maintaining the well-being and satisfaction of both our clients and our care staff.
Key Responsibilities:
Develop and manage individualized care plans for clients, ensuring they receive appropriate services.
Coordinate with healthcare professionals, social workers, and families to provide comprehensive care.
Monitor and assess the ongoing needs of clients, adjusting care plans as necessary.
Ensure compliance with all relevant regulations and best practices in care delivery.
Maintain accurate and up-to-date records of client care and progress.
Support care staff by providing guidance, training, and supervision.
Address any issues or concerns raised by clients or their families promptly and effectively.
Participate in regular team meetings and contribute to the continuous improvement of care services.
Manage schedules and allocate resources to ensure efficient and effective care delivery.
Requirements:
Bachelor’s degree in Social Work, Nursing, Healthcare Management, or a related field.
Proven experience in a care coordination or similar role.
Strong understanding of care planning, case management, and healthcare regulations.
Excellent communication and interpersonal skills.
Ability to work effectively in a team and independently.
Strong organizational and problem-solving skills.
Compassion, patience, and a commitment to providing high-quality care.
Proficiency in using care management software and other relevant technology.
CPR and First Aid certification is a plus.
